RIDGEWAY, SOUTH CAROLINA, JUL 22 – Southwinds Assisted Living failed multiple inspections from 2024 to 2025, with violations including improper medication handling and lack of trained staff, leading to its permanent closure by authorities.
- Southwinds Assisted Living on Humphries Road in Ridgeway was permanently closed by state health officials following multiple inspections that identified violations from February 2024 through July 2025.
- The department acted following complaints that raised concerns about resident care, and Director JoMonica Taylor stated they can recommend emergency suspension if residents face immediate health or safety threats.
- Inspections found issues including improper medication storage, outdated care plans, missing medications, locked doors, and residents housed beyond licensed capacity without trained supervision.
- DPH revoked Southwinds’ license on July 15, ordered all residents to be safely relocated, and the Kershaw County Sheriff's Office confirmed no criminal charges are planned while DPH leads the investigation.
