QUEENS COUNTY, PRINCE EDWARD ISLAND, JUL 23 – The Transportation Safety Board will examine systemic safety gaps after nearly 400 commercial vessel fires, including the MV Holiday Island blaze caused by failed fuel injection repairs.
- On July 23, 2025, the Transportation Safety Board of Canada issued its final report and opened a national probe into ship fires, according to The Canadian Press.
- A failed temporary repair to the Holiday Island’s fuel injection system sparked the July 2022 fire, investigators found, and the report found Transport Canada retained safety accountability as the vessel's 'authorized representative' despite Northumberland Ferries operating it.
- On July 22, 2022, at around 11 a.m. AT, the fire erupted in the ferry's engine room during its crossing, the suppression unit failed to discharge, and the compartment's incomplete seal allowed oxygen to fuel the blaze.
- The 50-year-old vessel was scrapped early due to extensive damage, after more than 200 people evacuated safely, and the crew and first responders contained the fire to the engine room.
- Its national investigation will probe systemic ship fire safety gaps, and the board noted that over 35 years it has issued nine fire safety concerns and 10 recommendations, said the investigation will examine systemic safety issues.
