Missing or failing track screws caused a GO train derailment at a key network junction, forcing platform closures and refunds for affected passengers, Metrolinx said.
- A GO train partially derailed on Feb. 2 as it left Union Station, with Lindsay saying missing screws caused the incident, disrupting service for days.
- A preliminary probe found nine screws sheared under fatigue, and Metrolinx noted track plates were secured with two screws instead of the required four, against track standards last updated in 2016.
- Not held down properly, the track shifted by just under three centimetres, the back of the GO train struck a track switch damaging signalling infrastructure, and four platforms at Union Station were taken out of service.
- Metrolinx has heightened inspections, checked for missing screws, implemented fixes, and issued automatic refunds for affected riders earlier this month, with refunds arriving within five to 10 business days.
- Metrolinx is pressing Toronto Terminals Railway for answers and investigating communication protocols, Lindsay assured `I can offer, again, a very confident assurance that the track has been inspected and is safe`.
