US Officials Blame Air India Captain for Crash

AHMEDABAD, GUJARAT, INDIA, JUL 21 – Investigations show the Air India captain deliberately cut engine fuel supply seconds after takeoff, causing engine failure that led to the crash killing 260 people, officials said.

  • Pilots flipping engine fuel switches caused two disasters: the Jeju Air crash at Muan Airport and the Air India AI171 crash in Ahmedabad, according to the Aircraft Accident Investigation Bureau on July 8, 2025.
  • In the wake of the two crashes, aviation figures called for cockpit CCTV, while pilots would resist additional monitoring, according to Terry Tozer.
  • A preliminary AAIB report found engine fuel control switches moved to cutoff, supported by cockpit voice recordings where one pilot asked “Why did you cut off?” and the other replied “I didn’t.”
  • The DGCA's July 14 order required airlines to inspect fuel switch locking mechanisms by July 21, and Air India completed checks on July 22 with no issues, according to the airline.
  • Experts note a similar TCMA failure in 2019 prompted calls for deeper software reviews, urging a comprehensive investigation into Boeing 787 systems.
