Woman Fed Dog Food by Portsmouth Care Workers Before Death
The ombudsman said the council’s commissioned care caused significant personal injustice and recommended an apology, fee refund and a £1,000 payment.
The Local Government and Social Care Ombudsman found "abysmal failures" in care provided by Radis Community Care to an elderly Portsmouth woman, including staff feeding her microwaved dog food recorded as a "lamb dinner."
Portsmouth City Council's safeguarding inquiry revealed a "catalogue of errors," with allegations that staff falsified records, ignored care plans, and left the woman unattended for prolonged periods in darkness overnight.
Care workers fed the woman three mouthfuls of vacuum-packed dog food, while her "Do Not Attempt Resuscitation" document was found scrunched up in a cupboard, unknown to staff.
