Coroner Finds Preventable Failures in Warrington Home Birth Service
A shortage of midwives and equipment failures delayed transfer to hospital, causing brain injury and death of newborn Pippa Gillibrand, coroner finds.
- On September 5, 2024, Cheshire coroner Victoria Davies concluded that `Pippa Gillibrand died as a result of a brain injury sustained due to an avoidable delay in her delivery`, Ms Davies said.
- The home birth team from Warrington and Halton Hospitals NHS Foundation Trust was already at another labour when Victoria Gillibrand went into labour on 25 August 2024, and the coroner found missed opportunities to suspend the service and advise hospital transfer.
- Midwife arrival at about 8.15am brought incorrect equipment and laptop issues; after 9am monitoring checks every five minutes lapsed and a decision to transfer at 10:00 led to forceps delivery at Warrington Hospital.
- Ms Davies said she will write to Health Secretary Wes Streeting, NHS England and NICE to raise concerns, while Warrington and Halton Teaching Hospitals NHS Foundation Trust accepted findings and remodelled its service; outside court, Victoria and Tom Gillibrand said they were failed and demanded change.
- Given community midwives assisted about three labours a year, Coroner Victoria Davies said she will issue a report to raise concerns about lack of national guidance and data collection, following a Manchester coroner's previous prevention report last year.
