- Provincial occupational safety officers Timothy Crowe and Clay Fredin identified critical deficiencies during a December inspection, including a missing exposure control plan, lack of joint health and safety committee, and unidentified hazardous piping carrying chlorine.
- The public works department, which employs between 42 and 60 staff, failed to provide adequate chlorine safety training or evacuation protocols, leaving workers unaware of procedures during a potential gas release.
- Focusing on strengthening safety programming, the City is not contesting the penalty and is formalizing respiratory protection, conducting emergency drills, and establishing a health and safety committee for public works.
- WorkSafeBC confirmed in April that the City achieved full compliance with orders by providing necessary information, education, and hiring additional supervisor positions to oversee ongoing safety training.
