- Affected lots can be identified by listed lot numbers; affected pads were distributed in the U.S., Puerto Rico, and Japan before March 2026, and customers were notified by mail on Mar. 2.
- The recall notice warned that the pads may cause local and systemic infections, including life‑threatening and central nervous system infections, and at‑risk patients, including cancer patients receiving chemotherapy and people with poorly controlled diabetes, face heightened risk.
- Cardinal Health told customers to separate and quarantine affected product upon inventory review and to notify downstream customers while disseminating the notice to all departments, clinics and external campuses.
- To confirm receipt, customers must return an acknowledgment form via fax to 614-652-9648 or email GMB-FieldCorrectiveAction@cardinalhealth.com, or call 800-292-9332 for questions.
